All SU2 Permission Slips are now being centralized and sent to one email address.

Send ONE copy of every permission at least two week prior to the event to SU2's to the following email address

2005 Permission Slip- Adobe Version - This form can by filled in on-line by clicking on each field. Print for your records. You will need full version of Abobe Acrobat to save your file. With Acrobat Reader you can only fill in and print. You can not save or email the file. If you don't have Adobe Acrobat (not Adobe Reader), use the Word version below.

When
to do them

Troop Meetings: Usually the first permission slip turned in each year is for regular troop meetings. An unsigned copy must be submitted to the SU and a signed copy must be turned in by each scout prior to or at the time of the first troop meeting and that signed copy is kept on file by the leader for one year.

SU Events, Community Events, Council events, campouts, excursions, field trips etc.: Any time the troop or a group of Girl Scouts from within the troop participate in any Girl Scout activity outside of regularly scheduled meetings, a signed permission slip is required from every participating Girl Scout.

  • At least two weeks prior to the event, an unsigned copy of the permission slip should be emailed to SU2's Permission Slip Supervisor, Terrie Mohlman at ps@ylsu2.org.
  • Leaders should distribute permission slips to every girl attending the event far enough ahead of time to give parents time to return the signed form.
  • A leader needs to have a signed permission slip from every Girl Scout attending the event. A scout without a signed permission is not allowed to attend a Girl Scout event. This is for insurance reasons.
  • Leaders and/or responsible adults need to keep the signed permission slips with them on scouting events.
    Remember, this is your permission to obtain medical assistance for a Girl Scout if necessary.

Product Sales: All girls participating in Fall Product or Cookie sales must submit a signed permission slip. This is not the standard permission slip but is specific to product sales and it is returned to the troop product sales manager. This permission slip is not filed with the SU permission slip manager.
